Requesting an Advanced Elective Schedule Change

 

What This Is
This form allows you to request a formal review of your placement in an Advanced Elective class. It ensures fairness, accountability, and helps teachers and counselors understand your situation.

When and Why to Use This Form

  • Significant Learning Concerns – You’ve tried to resolve issues with your teacher but still feel the class isn’t meeting your learning needs.

  • Scheduling Conflicts – There’s a documented conflict that prevents you from fully participating in the current elective.

  • Readiness or Placement Concerns – You feel you’re not ready for the advanced level or believe another class would better match your goals and skills.

  • Special Circumstances – A family or personal situation is affecting your ability to succeed in the current class.

Tip: Always talk to your teacher or counselor first. Many issues can be solved without changing classes. This form is for situations where you’ve already taken steps and still believe a change is necessary.

 

How to Complete the Request

  1. Get the Form
    Ask your teacher or counselor for the link or scan the QR code they provide to access the PDF form. The form is not posted publicly online.

  2. Fill Out the Form
    Open the PDF on your iPad using Apple’s built-in PDF reader or Markup tool. Type directly into the boxes, or print and write by hand. Complete all sections including Student Information, Reason for Request, Parent/Guardian Statement, and Student Agreement.

  3. Write Your Essay
    Create a one-page essay explaining:
    • Why you’re requesting a class change
    • What you’ve already tried to resolve the issue
    • How you’ll succeed if the change is approved

  4. Meet with Your Teacher or Counselor
    After you complete the form and essay, contact your teacher or counselor. In some cases you may be asked to participate in a short sit-down interview to discuss your request and plan for success.

  5. Sign the Form
    Use the built-in Markup tool on your iPad to draw your signature, or print and sign by hand. Your parent/guardian must sign as well.

  6. Submit the Form
    Return the completed form and essay directly to your teacher or counselor by email or in person (they’ll give you instructions). Keep participating in your current class until you receive official notice of any change.

Important Notes

  • No Guarantees – Completing this form does not automatically change your class placement.

  • Stay in Your Class – Continue all assignments and participation until an official change is made.

  • Be Complete – Missing signatures or essays may delay or deny your request.
